We are a patient-centered medical home, where you will be provided with compassionate, high quality, and evidence-based care. The FQHC designation allows us to provide quality care at an affordable price to everyone in our community. SFCHC is located at the corner of Mott Street and Chatsworth Avenue, near public transportation and freeways with ample free parking available. IMPORTANT: Our team is following all current Los Angeles County and state of California health & safety guidelines.

SFCHC accepts Medicare, Medi-Cal HMO, FamPact and other commercial plans. Services are available on a sliding fee scale which is determined by income and family size. If you have any questions about insurance or payment options, please call our office. Our friendly staff is happy to help you.

2023 Community Health Needs Assessment

We are pleased to share the SFCHC Board-approved “2023 Community Health Needs Assessment” (CHNA) conducted on behalf of the Center. As a federally qualified health center serving the most vulnerable, we are uniquely positioned to expand our services and overcome barriers to health equity – now and into the future. The data collected will help guide our future strategic planning activities, funding development and program growth. It also confirms the significant need for patients in our service area to receive high-quality, compassionate and culturally sensitive care that empowers them to make the best medical decisions. We are grateful for everyone's support as we continue SFCHC’s nearly century-long legacy of serving as a treasured healthcare home to the community we love.
