Referral Coordination Department: A Guiding Hand for Patients

For many, navigating a referral to see a healthcare specialist can seem complex and overwhelming. That’s why SFCHC’s Referral Coordination Department offers assistance every step of the way. Our team of five referral coordinators plays a key role in guiding both patients and healthcare providers through this journey, so individuals receive the care they need at the time they need it most. 

“We recognize the referral process can be challenging for patients and San Fernando Community Health Center has placed a priority on ensuring great support through the process,” said SFCHC Chief Medical Officer Dr. Andre Basmadzhyan, DO, Family Practice. “We have a dedicated and diligent referral coordination team who strives to facilitate timely access to outside specialists for our patients through prompt processing, interdepartmental coordination – such as with case management, providers and outreach – and with follow through.”   

The process begins when a healthcare provider requests a patient to receive specialty care, such as Cardiology, Orthopedics, Urology and Gastroenterology, among other areas of medicine. Then, SFCHC’s referral coordinators step in to manage the process and help patients understand exactly what to expect. From scheduling appointments to addressing insurance concerns, they ensure patients are well-informed and fully prepared for their appointments. 

SFCHC’s referral coordinators also work diligently to avoid delays, facilitate communication between specialty and primary care teams, and ensure all necessary documentation is in place. After the referral, they follow up with patients to review any additional steps on their care journey. This proactive approach removes barriers that could hinder a patient’s access to care. 

For example, logistical challenges and language issues can complicate a person’s ability to receive medical attention. In response, SFCHC’s referral coordinators collaborate with case managers to arrange necessary services such as transportation and interpreter assistance. They also work with the clinic’s Outreach Department to address insurance changes, so patients have a more positive and accessible healthcare experience. 

“Our primary objective is to minimize delays, eliminate confusion and ensure patients feel supported from start to finish,” said SFCHC Referral Coordinator Supervisor Marisol Hernandez. “By managing each step proactively, we reduce issues that my cause anxiety and empower patients to achieve better health outcomes.”

The success of SFCHC’s Referral Coordination Department relies on the dedication of skilled team members who possess strong communication and organizational skills, problem-solving abilities, attention to detail and empathy for patients. “Compassion and a genuine desire to assist patients are at the heart of our referral coordinator role,” Marisol said. “We focus on actively listening to patient needs and delivering a supportive, well-coordinated referral experience.”
